What is Fever?

Fever is also known as Pyrexia. It’s one of the most common presenting complaints in medical practice it is defined as the Human body temperature is that above the normal range, for that individual without any physiological cause to it.

in other words, temperature rise due to exercise or menstrual cycle doesn’t count as fever. this is usually above the 38*C in thermometer

Basically, Fever is not a disease that’s a symptom we our body caught by any Deases then our body shows a sign called Fever (Pyrexia) in Medical Term.

How to Make Nursing Care Plan for Pyrexia (Fever)

The First Point in Nursing Care Plan is Assessment in this Assessment the Assessment was in two Types

Subjective Data
Objective Data

Subjective Data in NSG

Data collected from a Pataint like a Patient said about his health and feelings about the disease is subjective data.

Objective Data in NSG

Data collected by a Nurse by observing the patient like patent behavior, His vital Sings, etc.

Nursing Care Plan for Fever (Assessment)

Subjective Data: The Patient says that “His body has Warm, Sweating, Weakness.”

Objective Data: The Patient has “Fever / Pyrexia”

Nursing Care Plan for Fever (Diagnosis)

Altered body Temperature related to disease condition as evidenced by taking Temperature

You can check patient body temperature by different methods

Nursing Care Plan for Fever (Goal)

To reduce body temperature.

In this Fever or pyrexia, our main priority is to decrease body temp.

Assess the Vital Sign
Provide cold sponging
Provide extra blanket
Provide keep and calm Environment
Provide Healthy Diet to the patient
Administer antipyretic medication as prescribed by a doctor

in the Intervention, the main motive is archiving the goal, and how you do that’s called Intervention

Assessed the Vital Sign

Provided Healthy Diet to the patient

In an implementation, we need to write all things that we have already done to reduce Fever.

The patient’s Temperature has been reduced.
The patient is now feeling well.

in the Evaluation, you need to write those things we got after the all implementation as a result
